3 Steps To File an Atlas or Day Tripper Claim

We want to ensure our clients receive prompt assistance in a medical emergency.  If you incur medical expenses, then here is what you need to do to file a claim:

1. If it’s not an emergency, then you must pre-notify any medical expenses or plan benefits by calling the 24 hour assistance telephone number which is listed on your ID card.  You need to notify the Assistance Company in advance (within 48 hours for emergencies) for any hospitalizations or surgeries as well as other items listed on your identification card or in the policy description. You may pre-certify through Client Zone or by phoning 1-800-605-2282 (outside USA call collect 24 hours a day 1-317-262-2132).

It is very important to pre-notify.  Failure to do so can result in a reduction of benefits.  All medical evacuation and emergency transportation arrangements have to be pre-notified.

2. In the event you need medical attention, present the identification card to your attending physician. The physician is welcome to submit bills directly to us. You will need to submit a Claimant’s Statement. To file a claim, please submit the original, itemized bills and payment receipts to us along with a Claimant’s Statement and Authorization form. You may obtain a Claimant’s Statement here. Claims must be filed within 60 days of receiving medical care.

3. Make a copy of your completed Claimant’s statement and receipts and then mail the originals to:

One last note, if you are injured or become ill while you are outside the US, then please have a doctor examine you and do not wait until you return home.  Eligible  illnesses or injuries initially treated or diagnosed overseas are also covered for any follow-up care after you return (subject to your deduductible and co-insurance).

Missionary on Furlough is Diagnosed with H1N1 “Swine Flu”

This morning a client of ours who is a misssionary in Japan but here in the US on furlough called me.  His family has only been here a couple weeks and his daughter was at summer camp when she became ill.  She was taken to a local Urgent Care center and was diagnosed with H1N1 A.K.A. the swine flu.
